Your Human Resources representative must provide you with a User Name and Password to access the MedBen websitefor enrollment. If you already have your User Name and Password, simply follow these enrollment instructions (PDF).
I prefer to enroll through the mail
Enrollment forms vary by state requirements. Please contact your Human Resources representative for the appropriate enrollment form.
